Anakin dies at the end of ROTJ because he has basically sworn off the Dark Side. He's no longer using it to stay alive without his breathing apparatus. When Jedi died they became “one with the Force” but not all became Force Ghosts. Becoming a Force Ghost was about basically completely letting go of the self.
