Tsunade actually will die of old age it's the only way to kill her. Essentially her regeneration jutsu ages her every time she uses it and she uses another jutsu to keep herself young. Let's put it this way she is 50 years old with the physical age of 90ish and is using medical ninjutsu to keep herself 30ish.
Is Tsunade really dead or still alive?
- As far as we know, Tsunade is still alive . After the Fourth Shinobi World War, Tsunade retires as the 5th Hokage, and she may have continued her tradition with leaving Konoha after the Second Shinobi World War when she retired as a Shinobi.

- Reputation: Tsunade has already established Sakura as her surrogate daughter. In Naruto, the relationship between teacher and student reflects that of parent and child. So in terms of character development, she doesn't need a child. But realistically, she could ofcourse still have a child.

- Deciding to tempt fate one more time, Tsunade made a bet with Naruto: if he could master the Rasengan in a week, therefore proving himself worthy of the position of Hokage, she would give him her necklace; if he failed, she wins his money-filled Gama-chan. He agreed and was able to accomplish the task, prompting Tsunade to put faith in the title of Hokage and the necklace one more time.
